In John's nightmare-verse, she didn't use the one bullet on Tanner; he's still alive and helping to push John towards his own death. That was too many words, though.

Sherlock is actually moved and touched by the realization that his death would devastate John completely (which he hasn't even processed yet, he's just doing the creepy genius-detective version of psychological first aid.)
